Step 1
Complete Medical Forms
A Medical Release Form for each child must be completed for registration. These forms are required by the Archdiocese of Cincinnati, even if you are doing home study.
Download the Adobe Acrobat Reader app to fill out the form electronically. It is a free download.
Once complete, please submit it using one of the following options:
Upload medical forms onto the online registration form in Step 2
Email each form to Mary Lou, mbaker@saint-max.org
Print out and bring to the parish office

The registration form for 2026-2027 will be open on Thursday, April 30, 2026.
Fuel and Spark Registration Fees 2026-2027
Early Bird fee (through 6/30/26): $70/child. Family Max $220
Regular fee (begins on 7/1/26) $95/child. Family Max $295.
